2019-2020 Kindergarten Registration

WHO: Any child who will be five years old prior to September 1, 2019, is a Heard County resident and did not attend our PreK program needs to register

WHERE: Centralhatchee Elementary- 315 Centralhatchee Parkway Franklin

Ephesus Elementary- 24414 GA Highway 100 Roopville

Heard County Elementary- 4647 Pearidge Road Franklin

WHEN: April 8th – April 19th – 8 am – 2:30 pm (Monday- Friday)

REQUIRED DOCUMENTATION:

• Parent/guardian identification (ex. driver’s license, passport)

• Child’s certified birth certificate

• Child’s social security card

• Parents’ proof of residency (ex. utility bill, rental agreement)

REQUIRED DOCUMENTATION PRIOR TO SCHOOL START:

• Child’s Immunization Record (GA Health Dept. Form 3231)

• Child’s Vision/Hearing/Dental/Nutrition Record (GA Health Dept. Form 3300 rev.2013)
